Delete a custom form in Outlook contacts 2003

I am trying to delete a custom form in Outlook 2003 that I have created. I do
not want it to show up any longer on the menu. I have located it in explorer
and deleted it, but it keeps coming back. Can these forms we create new in
design view be deleted or removed?

Explorer? Published forms don't show up in Windows Explorer. Delete them through the folder's Properties dialog.
