Default IE6 Header and Footer settings

Would someone please post their default Header and Footer parameters from
IE6 Page Setup?

After deleting mine and then trying to reproduce the original settings, the
Window Title and URL are getting chopped off with ellipsis "..." when there
is still plenty of room. They were >NOT< getting chopped off previously
with the default settings.

Here's what I tried:

Header: &w&b&bPage &p of &P
Footer: &u&b&b&d


Would have been nice if Microsoft would have included a Reset Defaults
option here.

As it happens, I saved the defaults because I can't leave settings like this
alone ;-)

IE Print settings
Header &w&bPage &p of &P
Footer &u&b&d
