Since that is a DoD form, it would probably be a copywright infringement to
create one. You might check the Government Printing Office site on line and
see if they have a down load available.
Actually, the opposite is most likely true. The government doesn't copyright
its output (some independent agencies do, but they are the small exception
to the rule). Unless it is Classified, it is Public Domain. That applies to
reports, forms, maps, images, etc. It is one of the larger benefits that the
government bestows upon the public as a whole.
