DB to Track Training Forms within an Office

Can anyone suggest a source for an existing sample database for tracking the
process of authorizing training forms within an organization? An optional,
secondary requirement is that each user's view should be limited to the
requests they submitted.
 
this would be easy to write.. i would contact (e-mail address removed) for a
small Access MDB type application

they really know what they're doing
 
ACS Group is a private consulting group. Would (e-mail address removed) be willing to
post a sample DB to this group or was the reply below meant as an
advertisement for the ACS Group?
 
it wasnt an advert; i was just reccomending someone to take this to.
