database search

I have used the database interface wizard to set up a database. I am trying
to do a search from 1 page and results from that search on another. I am able
to do a serch on one field but when I add a second field nothing shows up on
results page. What I am trying to do exactly is a search on a zip code and 1
of 3 options chosen. I have created several test records with a few different
zip codes and with one of the options. on my results page I am able to get
records to show by simply filtering the zip code but when I add the second
filter for the options, I get no records.

any ideas?
