Data Dictionary

I am looking for a decent Data Dictionary template which more or less
discloses Tables, columns, data types, lengths, whether a column is Nullable
or not, etc..

I found nothing within the Microsoft Template area.

Any help or guidance would be GREATLY appreciated!
