In Word jargon this is not a template but rather is what Word calls an
"online form." Check this in help. For more about online forms, follow the

Chapman's series of articles. You may also want to look at

Your form probably also should be a template, but that has a very different
meaning. For more on the different kinds of templates, tabs on the file new
