Custom Fields to behave like Recipient Fields

I am trying to create custom contact form which has additional information
in user defined fields. An example is to add the account manager details as
a reference to the contact. I would like this field to inherit all the
functionality of a "Recipient" field like To, CC or BCC such as being able to
select the name from address book etc., however I can only seem to create
fields of type "Text" and I don't know how to replicate the functionality of
the command button.

How should I approach this problem.

It is not possible to create such custom fields. You can, however, drag the To, Cc, or Bcc field from the All Mail Fields list to your contact form. You won't be able to change the caption, though.

Thanks for that Sue - you have saved me weeks of pulling my hair out trying
to work out how to do it!

Perhaps another option is to simulate the behaviour by adding some code to a
command button to enable the user to select a contact from the address book
and then populate the text field with the selection. How might this be
achieved?
