Creation our own custom form in outlook

Hi,

I am working on integration outlook into our applications.
I want to create the new outlook forms according to our specifications.

That form has to be called from .net and we have to assign the fields of new form from the .net code itself.
That form has to be placed in the Navigation pan of outlook


Please can any one help me regarding this problem.
