Creating Template for Index Tabs

I need to prepare a template for an Avery index label (#26089 1") b
1:00 CST and I have NO idea how to do it. Can someone please help?
The Avery website doesn't have a template for this label

Start with a ruler (the old wooden or plastic kind) and make a note of all
of the dimensions that you see if you select Details under the Label dialog.
The sort of info you need is sheet size, number across, number down, label
dimensions, top margin, left margin, space between vertically, space between
horizontally. Now modify an existing label details to match or create a
custom label.

rather doubt that they can be printed using most desktop printers. The
inserts are described as "write-on," which also suggests otherwise. If they
were designed to be machine printed, Avery would have included instructions
for setting up a custom label definition in Word. Products 16280, 16281,
16282, 16283 are described as "printable."

You might look at http://www.avery.com/us/products/tabbing/index.html, but
this is just a template for applying the tabs.
