Creating forms that will allow you to dump the completed form info into a worksheet

I'm wanting to create a form for a survey that could be complete
electronically either by attachment to e-mail or mailing a diskett
with the form saved on the diskette, then when it is sent back I coul
dump this information into a worksheet without re-keying. Th
information will then be sorted in vairous ways. I've tried to figur
out which of the software options in MS Office XP Professional would b
best to use and with everything I have read I'm still confussed. Woul
this be done better in Word for the form but then could the informatio
be dumped into Excel? I don't believe access would work because
couldn't send the form via a diskette. Any suggestions would be greatl
appreciated. If you do know what the best option would be, would yo
also know of a website that would help direct me in how to create it
that would be helpful also. Calling Microsoft was not that much help.
