creating a word template to transfer data

I would like to create a questionaire template and take the answers and
'populate' into a form letter. Any ideas how and if that is possible?

Possible, probably. Exactly how to best go about it depends on the details
of how you expect the work-flow to progress. Most elegant solutions would
require VBA programming (writing macros).

One simple scenario that would work without a macro would be to set the
template to "Save Data Only" (Tools/Options/Save). The result would be a
plain text file you could use for mail merge to the form letter.
