Creating a Table on Content

How do I creat a table of content that can be linked to each individual page
by selecting an item in from the table of content.
 
It is much easier to create a Table of Contents using the built-in heading
styles which show up in an automatic table of contents by default. However,
you can manually mark items for inclusion by selecting them and marking them
with TC fields. The first link below shows how to use both methods. The
