Creating a Switchboard and Linking it to a form

I am building a database to hold contact information with the client's phone,
fax, wiring inx, etc. and also linking these contacts to their account
number. I want to create a switchboard that will have multiple text fields
where someone could open up the program, the switchboard will pop up, they
can choose which piece of information they know, enter it, and then it will
jump to the form which will show all the extracted info that goes with that
piece of information (whether it be account number or maybe the name that
they used.) I believe i have to run a macro (or maybe a couple) for this to
be done. I have the form all set up and it's linked to the table and working
properly. Now I have to figure out how to get text boxes on the switchboard
that will run some sort of "search" function when the employee enters the
account # or name that they want to pull up information on. Can someone tell
me if they've done something similar, how I can get the text boxes on the
switchboard and how I can then have it run a search function to pull up the
info into for that criteria into the form that I built?
