Creating a fillable Form

I am trying to create a form in word that has areas for people to entered
disired information as well as have a few spots where they can type a
narrative. I would like to be able to lock the general spacing on the form
so when they type in their information the other information does not shift.
I have created the document as a form with inserts for the information
requested, the issue is that even though I have set the properties on some of
the fillable locations, the other locations shift as the form is completed.
I would appreciate any assistance you can provide.

Most forms designers use tables (they can be borderless) with exact row
height and preferred column width (auto resizing disabled).
