Creating a attachment entry field for an attachement

I have a template that I would like to add a attachment entry field for
files, etc. but am unsure how to add. Any assistance would be greatly
appreciated.
 
You mean a custom Outlook form? Leave the message body control on the form.
If you remove it, users can't add attachments.
