Created forms not showing new fields when sending - corporate ins.

I have amended a form in Outlook 2003 to include extra yes/no and text boxes.
When I select the form it looks ok, can fill out the boxes ok and send ok.
Unfortunately the recipient doesn't see any of the new boxes. This is a
corporate install and I have installed the Office 2003 update. When I "Save
As" I can only save to, C:\Documents and Settings\ihorne\Application
Data\Microsoft\Templates as I can't other libraries, and I don't get any
options to Publish.

Anyone have any ideas. It would be greatly appreciated!

The Publish command is on the Tools | Forms menu. Make sure you're following
all the steps necessary to ensure that the recipient gets your message in
rich-text format and has access to the form definition. The details depend
on your version of Outlook and mail configuration. Also the form may need a
