Create a form which allows another user to fill in information ...

I need to create a form which allows another user to fill in the blanks. The
underlining changes when typing into the document.
 
See the tutorials from Dian Chapman on "Please fill out this form", there
are 5 of them. The fifth one is about connecting your form to a database,
so you may not need that one.
