Countif or sumif for cumulative sums,pivot table & chart

Good day!!! New to the Community Forum. Thank you in advance for reviewing my
issue.
Background
Raw data (100's of entries as rows) entered into columns in Excel 2003:
Year Month Item Pass/Fail
08 Jan 1 Pass
08 Jan 1 Fail
08 Feb 2 Pass
..
..
..
09 Feb 2 Fail
09 Mar 1 Pass
I have created a Pivot Table with Year and Item as drop downs, Month and
Count of Pass/Fail as Columns respectively.
Would like to add columns to the original data to calculate:
1. the % of pass for each Item per Month
2. cumulative number of Items per Month
3. cumulative number of pass/fail for each Item per Month
4. cumulative % of Pass for each Item per Month
5. Include this calculated info in the Pivot Table
6. Use the Pivot Table to generate a dynamic chart by selecting Item and Year

I have tried combinations of Sumif, Countif, & Sumproduct. Looks like I do
not have enough bandwidth.
