Converting to a custom form

How do you convert and then import default forms (vCards) into custom forms
in Outlook 2003?
We used to be able to go to tools/forms and then to change forms and then
select the custom form in Outlook 2000.

No version of Outlook has ever supported importing to a custom form. You'd have to write custom code or use a third-party application. See http://www.outlookcode.com/d/customimport.htm.
