I suspect that this issue relates to the Outlook 2000 -2003 conversion.
You need to set the Outlook Contacts view to 'Detailed Address Cards' and
make sure that all the records display the full details.
Any that don't, open and save again from Outlook. This will correct the
display issue.
Then from Outlook > Tools > eMail accounts > View or Change address books.
Delete the Outlook address book.
Then add it again. This should repair the connection between address book
and contacts.
The right click the contacts folder in the Contacts task pane and check
properties > outlook address book ro ensure that there is a check in the
'show this folder as an e-mail address book' box.
Word should now be able to insert addresses from the address book.
