A client of mine worked for a company that programmed
forms right into the Outlook Contacts Address Card view
at the bottom of the page where a blank box normally
appears for typing in notes. All she had to do was click
on the icon (a box or triangle) next to the form name
(e.g. Letter of transmittal, Fax, etc.) and the form
opened with the contact information (name, address,
phone, etc.) and her signature (name, title, company)
already filled in.

Is this complicated? I am not a computer tech but am
usually computer-savy. Any suggestions?

Yes, it's complicated. It's hard to know from your description exactly what
they've done.
