Please help and Thank you in advance

I have a field as a unique Key made up of the: [Date(Short) "-" Last four of
SSN]
How do I set up in VB for Access a procudure or function to Take a date
enterd as short date and convert it to a number then take only the last four
of the SocialSecurity Number and combine them as "000000-0000"
