I want to end up with a query of table of:

Name Rank/Service1 Rank/Service2
Rank/Service3
Mary Smith 30 Case Mgt 1 Brain Injury
15 Autism
John Doe 54 Brain Injury 199 Autism
Bob Williams 50 Case Mgt

Any suggestions?

What will you do when you have more than 3 records for the same client?

You might be better off with a format like this:

Mary Smith

1 Brain Injury
15 Autism
30 Case Mgt

Total records for Mary Smith: 3 Highest Rank/Service: 1/Brain Injury

Bob Williams
