Code Module to Add In

Can a code module be "transformed" into an addin so that
when any instance of excel is opened a toolbar button
appears and when selected activates the code module?

Any ideas on how this is done????

Do you know of any example of creating a toolbar on open similar like
J-Walks one but for a toolbar instead of a menu?
