code for text box that will ID the user/modfier on a form

i am trying to figure out the code that i need to write in order to be able
to identify a user that last modified a form and a seperate box for the user
that created it. It don't matter if it is visible or not. I would like for
the box to identify the user by their login. I currently have securities in
place which require users to login.
