Checkbox fields

I am trying to send a mass e-mail collecting information on users would like to customize their reports. To do this, I am trying to send a message with numerous checkboxes to be clicked upon. However, I am trying to add one additional checkbox field which can be clicked upon when received by the recipient. Unfortunately, the clicked upon field does not appear clicked when received by the sender. I have the field unlocked in the compose page, but locked in the read page. Any thoughts?
 
Is the control bound to an Outlook property? This is required. Otherwise, Outlook has no place to save the data.
