Check Mark = Value

I performed all the following.
I then reran the Receive form. Same thing.
I went to the subForm and chose the combo-box for Service Codes.
It shows the other fields but nothing.
Is there something I should be adding to the subForm within the Receive
report.
BTW - the subform is in Datasheet view.
