Check boxes

Anybody who can help me will get my eternal thanks.

How can I insert check boxes to be ticked off (like in a form) in a normal
document.

Basically I want to have boxes that can be checked without having to have
the document be aform that is locked with all those inconveniences (can only
write in allowed zones, can not format text...)

Is this possible??
