Check boxes that disappear from on-line forms

When creating an on-line form for other to input data, the
check boxes used are functional during testing by the
author. When a user trys to check a check box, the box
disappears. Any suggestions?

Any chance you are using macrobutton fields instead of formfields? What you
are describing looks like what would happen if a document was separated from
its template using macrobutton checkboxes. The template contains AutoText
entries needed for the macro. There are a number of ways to handle
checkboxes in documents / templates. Take a look at the Checkbox template
