Check boxes replaced by exclamation marks!

I had set up a form with a number of check (tick) boxes and had previously printed copies successfully. Today, when I opened the file, the check boxes had been replaced by exclamation marks (!) and I don't know how to revert to the boxes.
 
Hi Tom,

Any hint as to the version of Word you are using and patches you have installed?

Additionally, how did you insert the check boxes? Did you use the Forms toolbar? The Control Toolbox? Insert a symbol?
