Capturing form data

Is it possible to capture form data when it is returns filled out? It would
be really great if it could be assembled in Excel or Access. I really
appreciate and help.

You didn't give us much in the way to go on, with regard to the details of
your scenario or the Outlook version(s) involved. So you might as well start
