Can't view form fields

I created a form that works just fine, but I added some new scrolling text
boxes and when I re-publish the form I can't view these recently added
fields. Any suggestions?

The text area form fields are there - List any medications ..., List
any...supplements..., Are you under medical..., Are you experiencing ...

However the page is not http://www.lindacleary.com/healthsurvey

- it is http://www.lindacleary.com/healthsurvey.html


The form at http://www.lindacleary.com/healthsurvey requires the
FrontPage extensions to work correctly, but extensions are not installed
on the website.
