Can't unload/remove bad com add-in

Environment : Windows XP with Visual Studio 2005 Pro with VSTO 2005 SE and
Outlook 2003.

I've been happily testing a com addin to detect new appointment items.. but
one time I obviously did something seriously wrong because Outlook popped up
a window ( and I wish I wrote the message down.. but it was something along
the lines of ) Outlook has detected a bad com addin would you like me to
unload it? By mistake I said yes... Now it seems I can't a) get my com add
in to load at startup, b) I've tried to remove it from
tools-options-other-advanced-com-addin , it goes away, but I still can't load
via VSTO debug.. c) an artifact is it has left a menu item on the main menu
bar..
I tried to repair outlook.. no go.. the I used the re-install option of the
installer.. again, my menu item is still there and I can't load coms.
I'm guessing I actually have to remove outlook and reinstall fresh? Any
suggestion on how to proceed? Thanks for taking the time to read this story

Outlook disabled the addin, it can only run again after it's re-enabled.
Help, About Microsoft Office Outlook, Disabled Items button, highlight the
addin in the disabled items dialog, click Enable, exit and restart Outlook.

Ken , thanks (once again) for your help.. I would have never looked under
'help/about...etc..' for this.. did as you said, and voila, I'm back in
business!
