Can't read memory card

Installed a new SD Micro card (with adaptor) from our cell phone into our
notebook. Windows says ". . .it must be formatted before it can be used."
Bummer, there are already pictures on the card. Tried to format, now Windows
says ". . .can not be formatted, check to see if device is installed
properly."

4.00 GB RAM
300 GB HDD
 
I had one of those last week that did that, turns out the adapter was bad.
I used a different adapter and it works fine.

Sounds correct. If I hold the card in a little deeper it then can read it.
