Can't Delete A Form

I know it is probably obvious but I can't see it - how do
I delete a form that I created in my Personal Forms
Library?

Not sure which Version you are using but in Outlook 2003 it is under Tools -
Options - Other tab - Advanced Options - Custom Forms - Manage Forms - Select
Personal Forms and all your forms should be displayed...now you can select
and delete the form you no longer need.
