Cannot enter new records in my form; Subforms unresponsive

Hi, I am looking for some help. I am designing a database to organize
data about companies and have structured it with five tables (two of
which are for the SubForms). Two of these tables, tblAssessments and
tblEnforcementSummary, are in one-to-many relationships with the two
tables that become SubForms in the form.

I thought I was finished creating a form that would be easy to use for
someone unfamiliar with the program. However, in doing my second
data-entry test, I discovered a series of problems. It seems that
these were spontaneous changes - the whole form worked just yesterday!

The problems:
1) A series of Controls have become unresponsive. When I try to enter
information, absolutely nothing happens.

2) When I click on two of my CheckBoxes, and when I try to tab (or
click) from those to the SubForms, I get a message saying that "You
cannot add or change a record because a related record is required in
the table tblFacilityInformation."

3) The AutoNumber fields in each of the subforms do not appear to be
working correctly (showing "AutoNumber" instead of the actual number).

When I try to save and close, I get the same message as above, followed
by the "You can't save this record at this time" error message.

I am at a loss as to how this happened, and how to fix it. I have
checked other posts and have made sure that Enabled says "yes", Locked
says "no", etc. etc. Any suggestions would be greatly appreciated!
