Calender & Drop down box

Hi all,

I can't seem to figure out how to create a custom form for Outlook that utilises the calender drop down and also drop down for time.

Basically, exactly how the standard Outlook appointment form looks like.

See the attached file.

p.s. using outlook2003

Please advise.
