CACHED EXCHANGE MODE STOPS WORKING

We have some machines running office 2003 via an exchange server.
Occasionally we find that e-mails are received by the server but are not
posted to the users mailbox. All e-mails previously received by the mail box
are secure but new ones seem not to get posted.
All user accounts are set for Cached exchange mode.
If we turn off cached exchange mode the missing e-mails appear. If we then
turn on cached exchange mode the e-mails may, or may not, then disappear.
Does any one know what causes this and what can be done to cure the problem?
This does not happen on Office 2k or earlier.

When the problem occurs, do the users see 'working offline' in the lower
right hand corner of the Outlook window?
Or, if the users click on another folder & then back to inbox, do the
messages appear?
Can they ping the Exchange server by name when the problem occurs?
What client OS are they using, and is there any personal/software firewall
installed?

Outlook 2003 is the only version of Outlook that supports cached mode
anyway, so that's somewhat moot.

I have this same problem. I have about 20 workstations still running fine in
cached mode, but I've had to disable cached mode on about 6 workstations
because the inbox stopped receiving new emails. I verified the automatic
send/receive is set (for about 5 minutes). When the problem happens,
clicking send/receive doesn't make any difference.

I haven't noticed the connected/offline status yet, I'll look for that.
Clicking on another folder doesn't change anything. The workstations have
Symantec AV CE, but no 3rd party firewall installed. I haven't tried pinging
the Exc server, but I know emails can be sent so the connection still exists.
The problem will happen at a random time, yesterday one stopped responding
between 2:22pm (last email in cached mode) and 2:35pm (next email without
cached mode).

The server is W2K SP4, EXC 2K SP3. The workstations are XP Pro SP1 or 2,
with Off2k3 SP1.
