button that prompt user for input

Hi

I want to create an outlook Button that when clicked prompt the user twice;

Prompt 1. Please enter Job Reference

Prompt 2. Please enter file name

Then it will take these inputted variables and place it into the subject with another string

e.g. user inputs 123132132 + photo.jpg

the subject show ++++123132132++++photo.jpg++++

This button should only appear on creating , replying or forwarding emails.
