Business Expense Report with GST calculation

I am looking for an expense report template which includes a GST sales tax component.
Any suggestions?

What is the template for exactly - employee expenses? I'm familiar with the UK tax system but I don't know much about US taxes I'm afraid. Have you tried searching through the templates in Excel? If you find one you like that doesn't have the GST component then we could try adapting it to include it.
