Bookmark in header

I am creating a form with a table (3 rows with form field name and blanks of
document reference info) at the top of the page that users are required to
fill in the blanks, therefore this table needs to appear on the document body
rather than the header (prefer user to fill in blanks from document body and
instead of entering data as header, this ensures instant identification of
document reference on 1st page ). I created a bookmark for this table and
insert (bookmark) in the header so that the table repeatedly appears on all
pages similar to a header. Now the table becomes duplicated on the first
page. Failed to insert bookmark from page 2 onwards, how can you hide the
bookmark on the page 1 or other way to achieve the purpose of this layout.
 
Each section can define a first page header, an even header, and an odd
header (and footers to match). You need to create a different first page
header that does not contain your bookmark.

File > Page Setup > Different First Page

Be aware of how many sections are in your document, and which section your
insertion point is in when you choose these commands, and what you have
selected in the Apply To list in the Page Setup dialog box.

You probably only want a different first page in the first section. You
probably don't need different headers and footers for even and odd pages.
