Blank check box

I am making a form to be printed and then filled in. How do I make a simple
blank check box? Is there a way without doing the equation thing. It just
confused me. Thanks,

You cannot resize it but it will print

You can resize it, sort of. You change the font size. This will change line
spacing though, unless you set your line spacing to exact.

Also, if you are to have the forms filled in on-line rather than printed &
filled in by hand the 'check' box should actually be called an 'X' box -
just so you won't be perplexed by its behavior :)
 
thanks, yes I guess it should be a X box but as long as it is a box that I
can mark to show completion that is fine with me. I am a pet sitter and get
tired of writing things over and over in my visit notes. I would much rather
check that they ate, drank, went for a walk, etc. I will give it a try.
