Where can I find an example of a basic access program to do interval billing
statements on a custom form, with balance carried forward, of a simple fee
for service, hourly rate, short list of services type of business?

There are a few templates that might get you started.

If you are doing this for someone else I'd suggest looking at MYOB.
It's as good as QuickBooks and Peachtree in most things and offers seamless
OBDC connectivity.
You do the forms and let MYOB handle the accounting.
 
Thanks, Mike. MYOB appears to be a more ambitious program than I want. I'm
wanting to customize to a specialized need (my wife's psychology practice).
Where might I look for the templates you mention?
