Well... you're asking in the wrong place. This is a newsgroup for technical
questions about Microsoft Access, a database program. I don't think you'll
find a Word template for something so specific, but perhaps you could ask in a
Word newsgroup. The webpage can be confusing - use the list of subjects on the
left of the screen, scroll down and find a group for Word.
