I downloaded 2033KB u6avi220.bin this afternoon into the Updates
folder, but there's no sign that it has installed. AVG is showing
virus database 384 from March 1st. Is there something I need to do?

It seems one of the necessary files is missing.

I've had this for the last 4 days. I have now updated sucessfully
(although it was a larger than normal update - 2Mb) so although I
haven't learnt anything or done anything different, at least the
problem wasn't permanent.

Don't forget (as I did) that you can download the file from the
website <URL:http://www.grisoft.com/> and do an update via the control
centre from disc.
