Avery 5412 Template

Hi, I need a template for the Avery 5412 labels. I have looked everywhere
and no one has one. They are very tiny labels. If anyone has one or knows
where to find one please help! Or can you tell me how to make one?

You will need to "roll your own" According to the Avery web site, those
labels are intended for hand written use.

Use a ruler to get the applicable dimensions and create a custom label to
suit.

I made one using MS WINWORD making a table with ROW=0.38 inch and COL=0.56
The table should be 10 rows by 10 columns. I then fill in the table and print it on a letter size page (8.5 x 11) but any size page larger than the label is ok. After I print the table on a letter size sheet, I overlay the avery 5412 label on top of the just printed table. When the alignment is perfect, I use transparent tape to tape down the edges of the avery label on four sides and then run the sheet with the label in place through the manual feed slot of my printer. A perfect 10 x 10 label. You can make up the table using MS EXCEL as 10 cols by 10 rows and copy and paste the EXCEL table into MS WINWORD. Once the table is in Word, adjust the table properties in word for 0.38 inch row by 0.46 inch column and then print.
