Appointment Calendar

Does anyone know of a calendar I can buy that customers can use, after
signing in with their User ID & Password, to reserve a block of time for an
appointment? There would have to have a place to put their name and phone
number for me to confirm the appointment before going to see them. I would
only need a monthly calendar which would automatically roll over to the next
month at the end of the current one. Thanks.

To further clarify, I need the calendar online and want to be able to
integrate it into FrontPage 2003.

Will require a custom server side script and a database
- both depend on what your server host supports (and your skills w/ server side scripting)
