an invoice template to request a payment

i am looking for a sample invoice, or template to be able to have an idea on
how to go about presenting one.
 
Open Word. Select File, New. Choose Templates on Microsoft.com and look at
their samples. Chances are you'll find something that closely meets your
needs.
