Address fields different on 2 pcs

Myself and another user are using the same verion of Outlook 2003 in the same
domain environment. For the contacts, however, when opening one to the
editting screen, on mine, under the "Addresses" section (on the General tab),
it shows a single white box where I can then type in the entire address.
However, for this other user, under "Addresses", it shows seperate fields
(white boxes) for the country, zip code, state, city, street...and in that
order too, not the other way around.

So, why is she getitng separate fields for each part of the address when no
one else is? Thanks.

Has someone installed a "custom form" on her system for that folder which
would explain it?

To check:

Method 1:
1) open a new contact on the machine with the separate addy fields - in the
title bar of the newly opened contact screen, if it says anything other then
"Untitled - Contact" - you're using a custom form

Method 2:

a) right-click on the folder in question -->
b) click on properties
c) click on "general tab" if not already open
d) if it says anything other then "IPM.Contact" in the "When posting to this
folder, use: " box - then a custom form is in force (i.e. Would contain a
value similar to "IPM.Contact.Something" - where the word "something" is the
name given to the form and is what would appear after the word "Untitled" as
mentioned in Method 1.
